Managers are too scared to intervene in cases of bullies. So they don't.

Look at how other industries approach the problem of reporting issues in a way that reduces the fear of retribution. The aviation, maritime trade, and rail sectors, all provide their staff with access to confidential reporting systems that use independent agencies to anonymise reports in order to find resolution. Surgical specialists benefit from such a system here in the UK, so it's not unfamiliar.
